WORLD NEWS ? USA Tropical Storm Isaac was expected to be at or near hurricane strength when it reaches the Florida Keys Sunday, according to the National Hurricane Center in Miami.

As of 2 a.m., Isaac had maximum sustained winds near 60 mph as it moved northwest at 17 mph about 265 miles east-southeast of Key West and 35 miles northeast of Canagua, Cuba.

Isaac was expected to strengthen over the next 48 hours and the latest NHC track showed it as a Category 1 hurricane as it approached the Keys around 8 p.m. Sunday.

What?s Open, What?s Closed Ahead of Isaac

The center of Isaac was expected to continue moving between the north coast of Cuba and Andros Island in the Bahamas, with tropical storm force winds extending outward up to 205 miles from the center.

A hurricane warning was in effect for the Florida Keys including the Dry Tortugas, the west coast of Florida from Bonita Beach southward to Ocean Reef, Florida Bay and Andros Island.

A hurricane watch was in effect for the Florida east coast from Golden Beach southward to Ocean Reef.

Shelters Open in Monroe County

The NHC issued a tropical storm warning for the Cuban provinces of Ciego De Avila, Sancti Spiritus, Villa Clara, Camaguey, Las Tunas, Granma, Holguin, Santiago De Cuba and Guantanamo, the Bahamas excluding Andros Island, Turks and Caicos Islands, the Florida east coast from Sebastian Inlet southward to Ocean Reef, Lake Okeechobee and the Florida west coast from north of Bonita Beach to Tarpon Springs, including Tampa Bay.

A tropical storm watch was in effect for the Cuban provinces of Matanzas and Cienfuegos, the Florida east coast north of Sebastian Inlet to Flagler Beach and the Florida west coast and the Florida Panhandle north of Tarpon Springs to Indian Pass.

On Saturday, Gov. Rick Scott declared a state of emergency for the state of Florida, while a hurricane watch was issued for Miami Beach and part of Miami-Dade. Broward was under a tropical storm warning.

South Floridians were urged to monitor the progress of Isaac throughout the weekend. Winds will increase locally early Sunday and peak during the afternoon and evening. Gale force winds were forecast to end Monday morning.

No School in Miami-Dade, Broward Monday

In addition to wind, abundant rain is expected with 4 to 8 inches in South Florida. There may also be higher amounts, perhaps up to 12 inches.

In Monroe County, officials strongly recommended that visitors leave the Keys if they can but no mandatory evacuation was issued.

In Broward, officials encouraged residents to brace for tropical storm conditions. Tropical storm conditions are also possible in portions of northwestern Cuba Sunday, the NHC said.

Hurricane Preparedness List

As for the Republican National Convention which was scheduled to begin Monday in Tampa, officials announced Saturday that the bulk of the convention would be delayed until Tuesday due to Isaac. RNC Chairman Reince Priebus said the convention will convene briefly Monday and recess until Tuesday afternoon.

Around 100 people demonstrated against a war with Iran in Tel Aviv on Thursday.

TEL AVIV, Israel -- A persistent group of anti-war protesters gathered in front Israel's Ministry of Defense in Central Tel Aviv on Thursday, chanting "We don't want another war!" and?warning the government against striking Iran's nuclear facilities.

Around 100 demonstrators carried signs declaring "No War" and "Don't bomb, talk!" Some lay on the ground and wrapped themselves in white sheets to simulate war casualties.

The demonstration, along with parallel actions in Haifa and Jerusalem, marked?the latest in a string of growing anti-war protests that has revealed?a deep unease many in Israel feel about a strike on Iran, despite Tehran's pursuit of nuclear technology.?

According to reports, Iran has installed many more uranium enrichment machines in an underground bunker, potentially paving the way for a significant expansion of work the West fears is ultimately aimed at making nuclear bombs.

NYT: UN chief heading to Iran despite US objections

While Tehran insists its nuclear program has peaceful aims, Israel sees the the work as a serious threat to its very existence. Israel has said it would take unilateral military action against Iran if the international community fails to stop any development of nuclear weapons. It is also widely assumed that Israel has nuclear weapons although it has never openly admitted to it.

New Iranian missiles have been put on display in Tehran - an exhibition that appears to be a warning to Israel. President Ahmadinejad says the short range missiles are meant for defense, not attack. But in Israel people are watching warily. NBC's John Ray reports.

According to insiders, the United States and other Western countries have told Israel that there is still time for diplomacy, and have urged the government of Prime Minister Benjamin Netanyahu to give sanctions and diplomatic pressure time to work.

Most oppose airstrikes
The protesters on Thursday said they were not blind to the threat Iran posed, but said violence was not the way to diffuse tensions.

"A military attack will not annihilate the Iranian nuclear problem. It will only delay it,"?Sharon Dolev, the director of the Israeli Disarmament Movement and one of the protest organizers, told NBC News.

Another protester, Naor Kapulnik, said he believes the nuclear problem in the Middle East originated in Israel.

"This is a war Israel has been planning for years to maintain their monopoly on nuclear weapons in the Middle East," he said. "They say that this is the way to protect and bring security to the Israeli Jewish citizens but I think that this is the exact opposite since Israel were the first to get nuclear weapons in the Middle East."

Kapulnik, Dolev and other protesters are not alone: According to the latest poll released by the independent think tank, Israel Democracy Institute, over 60 percent of Jewish Israelis oppose a strike on Iran.

The demonstrators also said they did not believe defense ministry estimates of the number of civilians who would be killed if war does break out. Officials have said that in the case of a war with?Iran and Lebanese Shiite militia Hezbollah, some 200 Israeli civilians would die. If?Syria joined forces with its regional allies, fatalities could rise to 300, according to the defense?ministry.?

"This is a very ridiculous thing to say. This is just one example of how they view the price of the war," Kapulnik, the demonstrator, said. ?

"Who will suffer the price? It's not going to (Defense Minister Ehud Barak); it will be the ordinary people walking on the streets. One civilian dead is one too many," he added.

'Determined to attack Iran'
Nevertheless, some members of the political establishment appear intent on a violent confrontation with Iran.?

Netanyahu "is determined to attack Iran before the U.S. presidential elections in November," Alon?Ben David, the military correspondent for Israel's Channel 10 News, told NBC News. ?

Not only that,?President Obama would have no choice but to back the Israeli strike, he said.

According to an Israeli TV news channel, Prime Minister Benjamin Netanyahu aims to strike Iran's nuclear facilities in the fall before the U.S. election, which would send oil soaring. Walter Zimmermann, United-ICAP, and John Kilduff, Again Capital, weigh in.

Dov Khenin, a member of Israel's Knesset for the Hadash party and a fixture of social justice and anti-war rallies in Tel Aviv, participated in the demonstration.

Israeli rhetoric on Iran strike heats up?

"I am here to protest against all the threats and ideas that a preemptive strike on Iran can be seen as some sort of solution to the problems we have here in the Middle East," he said. "Such a strike will cause a devastating regional war that can create havoc both here in Israel and the region around us."

Other prominent figures inside the Israeli establishment have expressed their opposition to an Israeli strike on Iran's nuclear facilities, including President Shimon Peres, former Director of Mossad Meir Dagan and opposition leader Shaul Mofaz.

But despite the voices calling for caution on many levels of Israeli society, it is the government that will vote on the strike -- and according to journalist Ben David, Natanyahu it has an "almost guaranteed" majority there.

Netanyahu's government recently persuaded Foreign Minister Avigdor Lieberman, who previously objected to attacking Iran, to support such a move. Netanyahu also reportedly dispatched a senior official to update Rabbi Ovadia Yosef, the elderly spiritual leader of the Shas ulta-orthodox coalition party, on the Iranian nuclear program in an attempt to win over Shas ministers' support for an attack.

On Friday Israel's Haaretz newspaper reported that Israel's security cabinet -- the body within the government responsible for?diplomatic negotiations and decisions in times of crisis -- is currently split evenly six to six on whether to bomb Iran. According to the paper, Netanyahu would almost certainly be able to garner a majority in support of such a move.?

Despite these behind-the-scenes moves, Dolev and other protesters insisted on having their voices heard.?

"There are other options that (the government) should put on the table if they are sincere. The option is talks," she said.?

The extent to which the Affordable Care Act succeeds in making affordable health insurance more widely available depends to a great degree on the success of the state-based health insurance exchanges that are currently being developed. A piece by Ewout van Ginneken and Katherine Schwartz in the latest New England Journal of Medicine offers some advice and cautions about the exchanges, based on the experiences of the Netherlands and Switzerland. Both those countries rely extensively on private insurers plus substantial government involvement.

Risk adjustment is one key to exchanges? success. To avoid the problem of cherry-picking, in which insurers try to hold down their costs by offering policies to only the healthiest applicants, risk adjustment redistributes premium dollars in an attempt to ensure that more of the money goes to insurers with the highest-risk patients. Van Ginneken and Schwartz report that effective risk-adjustment systems are feasible but require thought and good data:

The Dutch and Swiss reforms provide a second cautionary lesson for state exchanges. States at the forefront of creating exchanges are just now considering risk-adjustment mechanisms to reduce insurers? incentives to attract healthier enrollees and avoid sick people. The Dutch and Swiss experience indicates how critical a good risk-adjustment formula is. Until January, the Swiss risk-adjustment system had been relatively crude and ineffective: there were large premium differences both within and among cantons. Switzerland has just added prior hospitalization as an adjustment factor, and adjustments for patients? medical conditions will follow. The Dutch have a more sophisticated formula based on 20 years? experience, which thus far has discouraged insurers from engaging in risk selection.

Both countries also struggled initially with increases in the proportions of uninsured people, due to problems with enforcing the requirement that everyone must obtain coverage, and additional government measures were required. The U.S. insurance system is more complex, because the exchanges will function alongside employer-sponsored coverage, plans purchased outside the exchanges, and public coverage (Medicaid, the Children?s Health Insurance Program, and Medicare). This intricacy makes the challenges of implementing risk adjustment, assessing eligibility, enforcing the mandate, and administering subsidies more difficult and complex. However, the Dutch experience, especially, shows that with good data, solutions to these problems are technically feasible.

The piece also reminds us that exchanges alone are not the solution to US healthcare problems:

A sobering message is inescapable: although exchanges will help greatly to cover previously uninsured people, cost containment and quality improvements are not outcomes to be expected from the exchanges alone. The experience of Switzerland and the Netherlands suggests that reforms involving the provision and purchasing of health care are needed along with health insurance exchanges. Massachusetts, which established the first exchange in the United States, recognizes this lesson. Continuing in this direction, just 3 weeks ago (on July 31), the Massachusetts legislature passed a bill that establishes a commission to monitor the growth in health care costs, creating incentives for hospitals and other providers to reduce costs, and encourages the creation of accountable care organizations.

Creating exchanges is just the beginning of reforms ? involving all three markets (health insurance, health care purchasing, and health care provision) ? that are needed to achieve greater efficiency and quality in health care. As the Swiss and Dutch experience demonstrates, these reforms take time and require midcourse corrections, and we should expect them to be a work in progress for many years to come, with uncertain outcomes.

The Affordable Care Act?s main achievement has always been its plan for slashing our country?s shameful rates of uninsurance and underinsurance. (As I?ve said before, the ACA is not the ideal solution to these problems, but it?s the best we could get.) It does contain several provisions that could start us on a path toward paying for healthcare quality rather than quantity, but healthcare cost containment is an issue we?ll have to keep working on. Setting up a system that rewards insurers for health-improving innovations rather than risk-shifting innovations can help, but it will require strong and effective health insurance exchanges.
